Mounjaro has produced some of the most impressive weight loss results of any medication in clinical trials. But clinical trial averages and individual real-world results are different things. Here's what the data shows and what you should realistically expect.

What the clinical trials showed

SURMOUNT-1 (the pivotal obesity trial): 2,539 adults with obesity, no diabetes. Over 72 weeks at the maximum 15mg dose, participants lost an average of 22.5% of their body weight. At the 10mg dose, the average was 21.4%, and at 5mg it was 15.0%. The placebo group lost 3.1%.1

SURMOUNT-5 (head-to-head vs semaglutide): At maximum tolerated doses over 72 weeks, tirzepatide produced 20.2% weight loss compared to 13.7% for semaglutide 2.4mg.2

In practical terms, for someone starting at 100kg (~15st 10lb), a 22.5% loss would mean losing approximately 22.5kg (~3st 7lb), reaching roughly 77.5kg (~12st 2lb) after 18 months.

Real-world results vs clinical trials

Real-world results are typically somewhat lower than clinical trial results. This is normal and expected — clinical trial participants receive regular monitoring, structured support, and consistent dose escalation that may not be perfectly replicated in everyday clinical practice. Some real-world data from UK providers suggests average weight loss of around 15–20% over 12 months, which is still excellent.3

What affects your results?

  • Starting weight: People with higher starting weights tend to lose more in absolute terms
  • Dose reached: Higher doses produce more weight loss — reaching 15mg produces more than staying at 5mg or 10mg
  • Diet and exercise: Mounjaro works best alongside a reduced-calorie diet and increased physical activity. The medication reduces appetite; what you eat during that reduced appetite still matters
  • Adherence: Missing doses or inconsistent timing reduces effectiveness
  • Individual biology: Genetic factors, metabolic rate, hormonal profile, and other medications all affect how your body responds. Some people are "super-responders" who lose 30%+; others may lose 10%
  • Sex: The SURMOUNT-5 trial noted that weight loss was approximately 6% lower in men than women in both treatment groups
  • Duration: Most weight loss occurs in the first 9–12 months, then plateaus

Weight loss milestones from SURMOUNT-1

At the 15mg dose over 72 weeks:1

  • 96% of participants lost at least 5% of body weight
  • 90% lost at least 10%
  • 78% lost at least 15%
  • 63% lost at least 20%
  • 36% lost at least 25%

These numbers show that while the average is 22.5%, there's a wide spread. Most people achieve at least 10% (which is clinically meaningful), and about a third achieve 25% or more.

When does the weight loss happen?

Weight loss on Mounjaro follows a typical pattern: minimal loss in the first 4 weeks (the 2.5mg starting dose is primarily for tolerability), accelerating from weeks 5–8 as therapeutic doses are reached, fastest loss between months 3–9, then gradually slowing toward a plateau by months 12–18. Managing expectations

Go in expecting meaningful, life-changing weight loss — but not overnight transformation. The first month will feel slow. Real results build over months. If you're not losing as much as you expected, discuss with your prescriber whether a dose adjustment might help. And remember: even a 10% weight loss significantly reduces your risk of Type 2 diabetes, heart disease, and many other weight-related conditions.
